Who your doctor is. What you would want done. It is not a legal document. It is a plain record, in your own words, so nobody is left guessing.

Most people mean to get to it and never do, because it feels like a big job. It is not. Wendy will sit down with you and work through it a section at a time, at your own pace. When it is finished you keep it, and you tell one or two people you trust where to find it.
